Skilled and Home Health Care Services
Nursing Services (RN, LPN) • Therapy Services (PT, OT, ST)
- Experienced Nurses provide wound care and VAC therapy, home infusion/IV, chemotherapy, pain management, CHF program, post-operative care and education, cardiological and oncological cases, and mental health care.
- Physical and Occupational Therapists provide individualized treatments and exercise programs for those with CVA, arthritis, osteoarthritis, joint replacement rehabilitative needs, neck and back pain, post surgical and other developments.
- Speech Therapists assess, diagnose, treat, and prevent disorders related to speech, language, cognitive communication, voice and swallowing.
Veteran Assistance Services
Our unique VetAssist Program works to get veterans and/or surviving spouses home care assistance benefits from the Department of Veterans Affairs (VA).
- Those who have served can receive assistance with activities of daily living to maintain their independence and dignity.
- You could qualify for the little known benefits of the VA Aid & Attendance Pension Benefit.
- We all assist you in the process of applying for, obtaining and providing you services under your VA benefits.
Behavioral Health
Our licensed Behavioral Specialists thoroughly assess and address behavioral needs such that an individual, his or her care team and community may confidently work together toward the individual’s goals and independence.
- Comprehensive assessments of individuals with behavioral health needs by licensed professionals.
- Development and revision of support plans, including strategies to support an individual in-home, employment, and community settings.
- Approach and intervention training for individuals with behavioral needs, as well as their staff and caregivers.

Residential Habilitation
Our Residential Habilitation Homes serve individuals over the age of 18, who have intellectual/developmental disabilities and emotional/behavioral disorders.

Community Participation Supports (CPS)
